Basketball Evaluation Sheet

The school has my physical evaluation form on file?

I have to deliver a copy to the XC team, and it is too late to get physical examination (unless someone says if I do a walk-in), but physically I had last year for basketball, and I believe a physical exam lasts 365 days, then I ask the school to use it? (sorry for the run-ons and bad grammar, I'm in a hurry)

In addition, a student must pass examination each year before school physics. I wonder if you will, but you can try and ask. I hope you say yes:)
